A New Patient downloadable form is available for you to print and fill out before your visit to Dr. Fydell’s Office.

If you currently wear eye glasses, please bring them along with a current written list of medications that you are taking (dosages not required).

Your time is valuable and as a courtesy to all patients, we encourage you to arrive for your appointment 5 minutes early to register and have the pre-tests administered by the optometric assistant prior to your examination with Dr. Fydell.

On occasion we can accommodate walk-in patients; however, it is preferred that appointments are pre-booked whenever possible.

We understand that sometimes missed appointments cannot be helped; however, this means that we are unable to schedule someone else. Therefore, there will be a ‘no show’ fee the second time a patient misses an appointment. Thank you in advance for your understanding regarding this matter.
